Available at Autauga Pharmacy are the Johnson and Johnson/Janssen Covid-19 Vaccine and Booster. This vaccine is available to people 18 years of age or older. Please call 334-358-1774 to schedule your appointment.

Please bring your ID, insurance card and Medicare (red, white and blue) card if applicable. If you are scheduling the Booster, please also bring your COVID-19 Vaccination Record Card. You do not have to be covered by insurance in order to receive the vaccine or booster, but your ID is required.

You may print out and complete the following forms prior to your visit:

• Pre-Vaccination Screening Form
• Administration Form
• Consent Form
